An employee opened the front door to a commercial dryer to add some garments. The dryer did not turn off when the door was opened. Garments in the dryer caught her arm, twisting it and tearing tendons in her hand.

On February 14, 2018, around 5:40 a.m., an employee was walking through the plant when a rope from an empty laundry sling tangled around her foot/ankle. The employee bent over attempting to remove the rope from her foot/ankle, lost her balance and fell to the ground. She was hospitalized for a hip fracture.

An employee fell while delivering clean linen at a customer's location, suffering a concussion and back, shoulder, and hand pain.

A route driver was unloading a truck, lost his footing, and fell out of the rear of the truck resulting in an elbow fracture/dislocation. The employee was hospitalized.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee was changing a copper ring on a press welder. The machine was actuated, and the employee's left thumb was pinched between its ring and its nipple. The thumb tip was amputated between the nail and the first knuckle.

An employee was unjamming a lift when their right leg was caught between a pneumatic ram and the frame assembly of the lift. The employee's leg was fractured.

An employee was delivering laundry at a vendor's loading dock. He went inside the delivery trailer to load empty bins and tripped on the trailer floor and fell inside the trailer. The surface inside the trailer was approximately 2 inches higher than the loading dock surface. The employee was hospitalized with a fractured hip.

An employee tore a muscle in his abdomen while unloading soiled linen carts.

An employee was mounting a forklift when they slipped off the forklift resulting in strained muscles in the abdominal area. The employee had ice on their shoe at the time.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
