An employee was checking a collection box and lost his balance as he stood up. As he attempted to jump the front of the collection box ditch, the impact of the landing fractured his lower left leg.

An employee was separating a valve stem from an actuator stem by removing two-piece split nuts that hold it together. The actuator shaft shifted, causing the top half of the split nut to impact and pinch the employee's right thumb against the frame. The employee was hospitalized.

An employee was performing loading operations on a tanker truck when they fell 11 feet and 2 inches from the truck to the ground between the caged gangway access and the truck, resulting in fractures to the left wrist, orbital cheek bone, and pelvis.

An employee was opening a hatch to check liquid sulfur draining into a pit. The liquid sulfur splashed onto the employee's hand and face, causing chemical burns.

The Conlan Company

An employee was removing 65-foot concrete wall braces using a powered industrial truck and a choker sling. He was holding a tag line at the end of the brace. When the brace was removed the balance was uneven. The brace cantilevered and seesawed the employee 20 feet into the air. He struck the ground, fracturing his pelvis.

Ski Windham Operating Corp.

An employee was traveling down the right side of a ski run. At the bottom, there is a drop called the water bar. The employee hit the water bar, crashed, and tumbled to the bottom of the run, suffering a back injury.

An employee was at a school function at a trampoline park. He was doing flips when he landed on his back, injuring his neck. He required surgery and hospitalization.

An employee was slipping pipe when it became stuck and the pipe snapped, striking the employee in the back and ribs. The employee sustained fractured ribs.

An employee was standing on an injector head feeding chain with a rope, when the rope broke, causing him to lose his balance and fall to the ground. The employee sustained fractures to his left elbow and three ribs.

Cyclone Drilling Inc.

An employee was pulling drill line from the drill spool while the top drive was not set on the docking pins. The deadman was unexpectedly released which caused the top drive to fall to the drill floor from a height of 30 feet. This caused the drill line that was being pulled out and laid on the ground to retract and strike an employee. The employee suffered fractures to his legs and neck.

An employee was conducting maintenance on a pipe and was using a T-bar tool to secure the retainer cap on a pump. The employee's left index finger was caught in the rotating pump, resulting in a partial amputation.

Kisatchie Midnight Express

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

CHRISTUS St. Frances Cabrini Hospital

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.
