An employee was using a pocket knife to cut the banding off a pallet of materials. The knife lacerated the employee's right calf. The employee was hospitalized and underwent surgery.

At 10:02 p.m. on March 13, 2022, an employee was unplugging a rotary valve (the chute was clogged and the metal and cloth guard was damaged). The machine was turned on while the employee's right-hand fingers were in the valve. The employee's ring and middle fingertips were amputated.

On March 16, 2015, an employee, reached into the discharge while inspecting the line to verify that it was running. This resulted in the amputation of two right-hand fingertips.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

The machine clamped down on his left index finger, degloving the top part of the finger and resulting in a partial amputation.

An employee was performing maintenance on a roll cutting machine in a warehouse. The employee was removing excess grease from the bearings when the machine's chain caught their right index finger. The fingertip was amputated by the chain and sprocket.

During fiberglass work, an employee mixed a methyl ethyl ketone peroxide (MEKP) catalyst with a cobalt chemical that they thought was a dye. A chemical reaction occurred, resulting in an explosion. The employee sustained second-degree burns on his back and a blister on his right arm.

An employee was walking when she tripped over a box on the ground and fell. The employee was hospitalized with a severe dislocation to the hip and leg that required surgery.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
