On or about December 04, 2015, a worker was working on a tie machine when a jam occurred. The worker called the injured employee for assistance. The injured employee disconnected the energy source, a pneumatic airline, and proceeded to clear the jam manually. As the employee tried to clear the jam, the machine's cutting knife was actuated, amputating the employee's right fingertip.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
