Olin Corporation - Winchester Division

On October 10, 2018, at approximately 7:36 p.m., three employees were working inside an ammunition manufacturing facility on a primer manufacturing line. The injured employee was operating a primer cup shaking unit when the motor stopped working. The employee was troubleshooting the motor and reached through a sliding guard to try and push it to get it going. When the pulley motor started, the employee suffered an avulsion injury to their right index finger. The fingertip was surgically amputated.

An employee was working on a feed system and attempting to reline the feed line to the machine when her right middle finger was pinched by the homing plate, amputating her fingertip.
