An employee was climbing a scaffold when the scaffold came apart. The employee fell 12-15 feet to the ground resulting in a upper left thigh laceration and a compression fracture to the T10 vertebra.
An employee was working 8 feet above the ground while installing sheet metal as a roof over a pathway. The employee secured one side of the sheet metal and stepped over it to secure the other side.
