An employee was attempting to line up two I-beams measuring 7 to 8 feet to get them ready to be moved by a forklift. As he slid the top I-beam over the other, the back end of the I-beam slipped, causing the front of the beam to contact the employee's left ring finger and cut the fingertip, resulting in an amputation.

An employee was working 8 feet above the ground while installing sheet metal as a roof over a pathway. The employee secured one side of the sheet metal and stepped over it to secure the other side. He fell through the sheet metal, hitting the ground on his left side and arm. The employee fractured ribs and a clavicle.

An employee was working under a truck. When he removed the drive shaft, the vehicle began to roll backwards. The truck's catalytic converter then struck his chest area, injuring his ribs and clavicle. He was hospitalized.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

An employee was cleaning pans containing adhesive by pouring acetone into the pans. After soaking, the employee used a plastic spatula to scrape the paint from the pan. During scraping, a fire spontaneously started underneath the employee near the pan. The employee threw the container of acetone out of the way to avoid it catching on fire, and spilled acetone onto his clothing, causing the fire to spread. The employee suffered burns to both hands and the right calf.
