An employee was lubricating equipment. He was using a rag to clean lubricant from the PT4 conveyor. The rag was caught in the chain and sprocket, which pulled in the employee's left hand crushing the middle finger in the chain and sprocket. The finger was amputated at the first knuckle at the hospital.
