An employee was clearing a jam on an energized packaging machine and amputated the end of the right thumb.
An employee was lubricating equipment. He was using a rag to clean lubricant from the PT4 conveyor. The rag was caught in the chain and sprocket, which pulled in the employee's left hand crushing the middle finger in the chain and sprocket. The finger was amputated at the first knuckle at the hospital.
