An employee was removing chuck from a lathe machine. When he operated the machine hydraulics by stepping on the foot pedal, the chuck did not dislodge completely. As he was troubleshooting the problem on the chuck, the machine was activated by the foot pedal. This caused the chuck to move back inside the machine, amputating is right-hand middle and ring fingertips.

An employee was operating a machine when the hydraulic clamp initiated, causing an injury to the employee's left ring finger that resulted in a fingertip amputation.
