An employee was operating a boring mill machine while his right hand rested on top of a metal beam on the shuttle. The beam moved during the operation of the machine and pinched his right middle fingertip between it and another metal beam. His fingertip was amputated.

An employee was making a cut on the metal of a rail car using an air arcing cutting torch. The employee's clothing was ignited, resulting in second-degree burns with a small patch of third-degree burns to the right side of his back and neck.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. The employee was placing a two-foot wooden brace (4 by 4) under the metal, which was being raised by a crane operator using a magnet. The magnet shifted and caused the load to catch the employee's right index fingertip against the wooden brace, resulting in a fingertip amputation.
