On February 14, 2016, an employee was scrapping down the sides of a 56 gallon mixing tank with a wooden paint stick. His shirt got tangled with the rotating mixer shaft and the employee was pulled against the tank. The employee suffered a punctured lung, cracked ribs and lacerations to his right side, forearm and forehead.
