An employee was evaluating an issue with a shaft in a winder. The employee's left index, middle, and ring fingers got caught between the belt drive and sprocket, resulting in one finger amputation at the first joint.

Two employees were installing a new plastic roll on a winder machine. One of them was pulling the film with her left hand from the output side of the machine; the film and the rotating member of the winder caught her left hand, breaking both bones in her left forearm.

An employee was drilling a hole in a can containing aerosol spray glue. The can exploded, causing second-degree burns to the employee's arms, face, and neck.

An employee was activating the pneumatic release button on the spindle of the winder when the spool spun and the employee's middle fingertip was caught against part of the spindle cradle and resulting in an avulsion.

An employee was descending a fixed caged ladder when he missed the top rung and began to fall. He grabbed onto a vertical member of the cage assembly to catch himself and caught his right pinky in a 1/2-inch gap between the upright and a toe board section. His right pinky fingertip was amputated.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.

An employee was passing through a large metal door when the door closed on the employee, resulting in a cracked rib.

An employee was preparing to cut lumber on a panel saw when the clamp engaged and caught the employee's left middle finger, resulting in a fingertip amputation.

A subcontracted engineer was advising maintenance how to make a modification to the embossing roll system on a new line. Maintenance was bringing the motor and gearbox down with a crane. The load shifted when it was a few inches from the ground. The engineer went to catch/maneuver the gearbox and the fingers on his left hand were crushed between the I-beam base of the gearbox and the concrete floor. A finger was amputated.

On July 30, 2025, an employee was assisting co-workers in using a hoist to position an injection molding tool onto a work table. As the tool was being lowered, the slide section shifted, trapping and amputating the employee's right thumb tip.

An employee was manually lifting a film roll when their right middle fingertip was pinched between the roll and a metal beam, resulting in a fingertip amputation.

At about 10:00 a.m. on July 13, 2025, an employee was cleaning the adhesive from running rollers using a rag soaked in cleaning solvent to prevent contamination. The rag was caught between two rollers and pulled the employee's right hand between the rollers. The employee sustained right hand crushing injuries, de-gloving injuries (palmer and dorsal sides), and fractures. The employee was hospitalized and required surgery. The machine was not locked out/tagged out at the time.

An employee working on a sheet line was removing masking from the underside of plastic sheeting that was being lifted by a vacuum lift. The plastic sheet measured 4 feet wide, 14 feet long, and approximately 1 inch thick, and it weighed approximately 300 pounds. The vacuum lost suction and dropped the plastic sheet onto the employee's arm, pinning it between two sheets and resulting in a broken right forearm.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
