Final narrative
An employee was operating a CNC machine that had a chip conveyor extending from it. The employee turned the CNC machine off to retrieve a chip ball piece of scrap from the conveyor. As he was retrieving the scrap, the machine was unintentionally activated. The employee's glove became caught in the moving parts and his fingers were caught where the conveyor meets the CNC machine. His left-hand index and middle fingers were partially amputated.
An employee was snipping the last banding on a bundle of steel bars that were being brought in by a feeder table conveyor. The steel bars fell onto the employee's little finger, resulting in a fingertip amputation.
