An employee was operating a 2,500-ton vertical press while the machine was in idle mode. The employee was manually spinning the die manipulator when his right middle fingertip was caught and amputated in a pinch point while spinning the die manipulator.

An employee was operating a CNC machine that had a chip conveyor extending from it. The employee turned the CNC machine off to retrieve a chip ball piece of scrap from the conveyor. As he was retrieving the scrap, the machine was unintentionally activated. The employee's glove became caught in the moving parts and his fingers were caught where the conveyor meets the CNC machine. His left-hand index and middle fingers were partially amputated.
