An employee was inside a security guard shack when a generator from a construction company that was performing work at the facility emitted exhaust fumes into the shack. The employee became dizzy and lightheaded, requiring hospitalization for carbon monoxide exposure.

An employee was operating a golf cart. The cart flipped on its side when the rear wheel made contact with a parking lot's raised curb. The employee's right ankle was pinned between the cart and the pavement, causing the ankle to break.
