On 3/8/2016, at approximately 2:00 PM, an employee was cutting up boxes. As the employee was attempting to step over the boxes, she tripped, fell, and hit her head. She lost consciousness and suffered a concussion, requiring hospitalization.
An employee was on a ladder that was on a counter changing a light bulb on the ceiling. As he descended the ladder, he fell sideways with the ladder and struck his head on the floor.
