An employee was herding cattle from the pens to the kill floor. On the way to the kill floor, the employee tripped and struck his head on the concrete of the pens. As the cattle passed him, one of their hoofs struck him in the ribs. He was hospitalized with broken ribs, a concussion, a broken nose, and a head laceration.

The suction side of this pump has a chemical O-ring that compresses the suction hose feeding the chemical into the pump. The O-ring was compromised, allowing chemical to drip out of the hose and onto the floor. The employee breathed in the chemical vapors, exposing his respiratory system to them.

When the employee grabbed the loose chain, a light sensor caused the machine to restart. This created a pinch point between the chain and sprocket that caught the employee's left little finger. The finger was surgically amputated at the second knuckle.
