On 3/22/16, at 5:15 PM, an employee was walking down the indoor employee entrance stairs when she fell and broke her left arm and leg.

An employee slipped on the second step of a stairway and fell to the concrete ground. The employee was hospitalized with a concussion and a subdermal hematoma.
An employee on a portable ladder was placing merchandise on an overhead shelf when she slipped and fell off the ladder. The employee sustained head and hip injuries.
