Federal OSHA recorded a severe workplace injury at TRUE MANUFACTURING, 2001 E. Terra Lane, O FALLON, MISSOURI 63366 on — Amputations, affecting the fingertip(s).

Two employees were performing maintenance on the Schlatter machine (high speed machine that produces wire shelving). One of the employees suffered the amputation of the left index fingertip.

TRUE MANUFACTURING

Three employees were reorganizing a work area by staging material-handling tables. Two employees were pushing a table while the third employee guided it. The table became stuck on the wheel of another nearby table and while the two employees tried to push it free, the third employee moved the other table. The newly freed table moved forward into the other table, and the third employee's right middle finger was caught between the two. A third of the fingertip was amputated.

TRUE MANUFACTURING

An employee was grinding a piece of metal using a grinding wheel. The wheel pulled the metal in and the distal tuft of the employee's left thumb was amputated.

TRUE MANUFACTURING

An employee was cutting plywood panels with a saw when he went to stack the finished panel. While he was stacking/leaning the upright panel against other panels, the stack of panels began to slide and knocked his legs out from under him. He landed on his left side and broke his hip, requiring hospitalization and surgery.

DGS Import, LLC

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Bauer Foundation Corp.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

Guggisberg Cheese, Inc.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

Alsco Inc.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

Thermal Concepts, LLC

An employee was working next to a 12-inch, 21-foot-long pipe that was being lowered using a 1-ton chain fall. The chain fall slipped, causing the steel pipe to drop. It struck his left hand, causing the partial amputation of the ring fingertip and the loss of the fingernail.

True Manufacturing Company

An employee was fabricating an evaporator fixture. The fixture was sitting on a table and the employee was utilizing a hoist and chain sling hooks to elevate one side of the fixture into position to weld a piece on it. The hoist moved slightly on the bridge trolley, creating slack in the chain that caused one of the legs of the sling to come out of the square tubing. The evaporator fixture came down onto the employee's right ring finger, pinning it against the table. The employee's finger was partially amputated and was later surgically amputated above the knuckle.

Trane

An employee was operating a shear when its blade came down and amputated his left middle and ring fingertips. The machine was guarded at the time.

Johnson Thermal Systems, Inc.

An employee was removing material from a pallet rack using a standup forklift when an upper shelf was knocked out of place causing a pallet of industrial fans to fall 12-15 feet. The employee was struck by the falling pallet. The employee sustained a laceration to the front-right of their head and fractures to three vertebrae.

WADDELL CONCRETE INC.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

Dollar Tree

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

Corrigan Brothers Inc

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

Plumb Supply Company

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.

JRI INDUSTRIES

An employee was manipulating dies on a brake press. The press caught and crushed the employee's right forearm, resulting in hospitalization.
