An employee was performing maintenance on the refrigeration unit of a truck. As they were walking up a platform ladder, the ladder fell backward and the employee reached up with his right hand to hold onto something. The employee contacted the refrigeration motor belt, which pulled his index and middle fingers into a pulley, resulting in amputations to the first knuckle.

An employee was unhooking an air hose from an oil pump in a shop. As he disconnected the quick coupler, the compressed air caused an abrasion to his right ring finger.

An employee was performing a maintenance check when the reefer engine turned back on as the employee was checking the belt tension. The employee suffered open fractures to their hand and one finger.

An employee tripped and fell in a concrete parking lot for rental vehicles, resulting in a broken femur.

An employee fell from a lift gate to the ground, resulting in a broken hip.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was working under a pickup truck, changing out the shift linkage cable. The cable was being attached to the gear shift from within the cab. When the cable was pulled within the cab to connect it, the truck went into neutral and rolled over the employee's left shoulder. The employee suffered fractures to the left humerus, left shoulder blade, and left-side ribs, as well as severe contusions to the left shoulder.

On June 13, 2025, an employee was using a jack stand to lift the front axle of a semi truck. When the jack settled, it slipped off the axle and kicked backward into the employee's leg. The employee suffered a tear in an artery in the leg, as well as a contusion and swelling.

An employee was on a forklift platform stocking material on a rack and fell approximately six feet and struck his head. The employee was hospitalized.

An employee was walking in the warehouse, picking parts for an order. The employee turned a corner and collided with another worker, head-to-head. One of the workers fell to the floor fracturing her left shoulder. The other worker was not injured.

On May 5, 2020, an employee was measuring a trailer to install a winch when he tripped and hit his head on the truck's bumper. He suffered a bruise on his left side and was hospitalized with a head injury.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
