An employee was descending the back stairwell to go to lunch when he missed a step. As he fell forward, his toes hit a step which pulled his right leg behind him. He landed on the concrete stairwell landing between floors. He sustained a partial rupture of the right quadriceps tendon and an avulsion fracture involving the right patella. The employee required surgery.

An employee was driving a 4-seat ATV from one field to the next field on the site. They went around a corner and glare from the sun blocked the employee's sight. The ATV struck a parked car. The employee sustained a broken hip and a laceration to the head.

An employee was talking to a patient when he lost his balance and fell down the stairs. The employee sustained injuries to both patellar tendons and required surgery.

An employee was helping a patient get out of bed and adjusting the bed rail when the patient became agitated and kicked the employee in the abdomen. The employee was hospitalized.

At about 4:00 p.m. on March 8, 2024, an employee was using a table saw and his hand contacted the blade, resulting in amputation of the tip of his thumb.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee slipped on water in a hospital hallway, fell, and landed on his left hip. He suffered a contusion and fracture to the hip.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

An employee was attaching a steel angle plate weighing (approximately 200 pounds) to the end of a structural member (5,000 pounds) and was supported by dunnage. The dunnage was a crane mat (11 inches tall) with a 4x6 board on top of it and it supported five identical members. The main piece was 30.75 feet long, 37.25 feet tall, and 1 food wide. As the employee slid the piece of metal on top of the member to begin attaching it, the entire member began to tip over. The employee went to stop the main piece from tipping over and it landed on his ankle, fracturing it.

An employee was preparing hot melted butter on a flat-top grill and placing it in small containers once it was melted. As she turned to set the container down, it slipped out of her hand and landed on the grill. Butter splashed up and hit her in the face. The employee suffered second-degree burns to her right eye and the right side of her head and face, as well as first-degree burns to her right forearm.

An employee pulled into the customer's driveway, parked, and exited the vehicle. He opened the sliding side door on the vehicle to retrieve a package. The customer began backing their vehicle out of their garage and it struck the employee in the back of his left leg. He was pinned him between the two vehicles, causing contusions and lacerations.

An employee reached into a hamper, picked up a package, and was hospitalized with a back injury that caused pain in his lower extremities.

On July 10, 2025, an employee tripped over a yellow pillar and fell to the floor. The employee was hospitalized with a fractured left femur.
