An employee was descending the back stairwell to go to lunch when he missed a step. As he fell forward, his toes hit a step which pulled his right leg behind him. He landed on the concrete stairwell landing between floors. He sustained a partial rupture of the right quadriceps tendon and an avulsion fracture involving the right patella. The employee required surgery.

An employee was talking to a patient when he lost his balance and fell down the stairs. The employee sustained injuries to both patellar tendons and required surgery.

An employee was helping a patient get out of bed and adjusting the bed rail when the patient became agitated and kicked the employee in the abdomen. The employee was hospitalized.

At about 4:00 p.m. on March 8, 2024, an employee was using a table saw and his hand contacted the blade, resulting in amputation of the tip of his thumb.

An employee was cutting a piece of metal trim using a miter saw when it kicked back, amputating his fingers.

An employee was operating a lift truck when it struck a stationary lift truck. His left thumb was pinched between the two lift trucks, resulting in a partial amputation.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was operating an electric pallet jack when it struck two parked forklifts. The impact caused the employee to be ejected from the pallet jack, and his right foot became trapped between the forklifts, resulting in a severe laceration to the foot.

An employee was operating the compaction levers on a truck when a motor vehicle crashed into the back of the stopped truck. The employee sustained fractured ribs and contusions.

On October 16, 2025, an employee operating an order selector was passing a moving scrubber when the scrubber suddenly stopped. The order selector struck the back of the scrubber, resulting in a cut to the employee's left ankle.

An employee was repairing the hydronic heating system for the building. The employee was standing on a 6-foot ladder to remove a three-way mixing valve from the ceiling pipes and change the valve stem. As the mixing valve was loosened, hot hydronic fluid (water/steam) poured out through the valve stem and splashed onto the employee's skin and clothing. The employee sustained second- and third-degree burns to their chest and stomach. They also sustained first- and second-degree burns on their left hand, arms, and thigh.

An employee was repairing a dump truck tailgate when a finger on his right hand was crushed between the metal base and the hook for the chain.

An employee was operating a drill press when their gloved hand got caught in the drill, resulting in an amputation of the top portion of his right index finger.

An employee was struck by a reversing excavator resulting in broken bones in the right leg.

An employee was standing on a burial vault lid approximately 3 to 4 feet below grade hand-shoveling soil off the lid. An excavator was at rest on ground level accepting the shovels of soil. The excavator bucket inadvertently moved, causing the boom to swing. The excavator bucket then pinned the employee's hip to the wall of the grave, resulting in a fractured pelvis.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
