An employee was on an A-frame ladder troubleshooting the sawmill conveyor system. He fell from the ladder and his left pinky finger was crushed by an unguarded chain and sprocket gear, requiring hospitalization.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
