On 23 February 2015, at approximately 1030 hrs at the Flogistix manufacturing site in Pampa, Texas, employee was cutting a metal stud on a chop saw when the material kicked back and crushed the distal phalanx of the index finger (right hand). The employee was taken to Pampa Memorial Hospital where physicians opted to amputate due to the extent of the injury.

As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt.
