Final narrative
On 4-19-16, an employee suffered the amputation of the distal tip of the left index finger. The tip was reattached and there was no bone separation. The injured worker placed his hand inside of a valve port to move metal shavings out of the way so that he would not damage the finish cut of a compressor. The injured worker is not sure if the shavings cut his finger or if it was the rotation of the bore mill machine that cut his finger.
Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.
