An employee was walking on top of a double-T form bed while carrying a piece of rebar. He stepped on a strip of foam that had been placed over a stem channel, and his leg went into the channel causing him to fall onto the steel bed. The rebar struck him. It lacerated the left side of his chin and broke his safety glasses, causing pieces of the glasses to enter his left eye.

An employee was pulling steel cables when one of the cables under tension broke loose and injured both of the employee's hands.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.
