Two employees were loading explosive pyrotechnics into a drying tumbler when the material exploded, burning the face and hands of both employees. One of the employees also fractured her elbow and lacerated her leg.

An employee was operating a tooling center machine behind a barricade inside of a building when an explosion of unknown source occurred. The explosion blew the front of the barricade inward, forcing the employee backward and pinning him to the floor. The employee sustained multiple traumas and both legs were broken.

An employee was sitting in the cab of the truck while waste oxidizer was being offloaded. The waste reacted with the receiving tank and an explosion occurred in the tank. Fire traveled through the hose to the tanker truck, causing it to explode. The employee jumped from the burning truck and was hospitalized with burns to his face, chest, and arms.

Complete Fire Protection Inc.

An employee was installing piping and valves for fire protection systems in the vicinity of an electrical meter outside of a new warehouse. The meter exploded, causing burns to the employee's face and arms.

MI Metals, Inc.

An employee was pouring molten aluminum into a drain pot. The aluminum touched moisture at the bottom of the drain pot and an explosion occurred. The employee sustained burns to their face, hands, and legs.

An employee was operating an oxygen-deficient oven that required manual ignition to make charcoal from wood. As the oven was opened, oxygen was introduced to the environment, resulting in an explosion. The employee sustained head trauma as well as fractures to the arms and legs.

An employee was setting up the electrical parts for a fireworks display. They were inserting an e-match when it generated friction with something else. A 3-inch firework shell ignited, injuring the employee's hand and head.

An employee was riding as a passenger in a golf cart on a concrete pathway. The cart hit a bump and the employee fell out, suffering a head injury.

An employee was clearing a firearm that had a jam in it. To disassemble the firearm, the employee was required to pull the trigger to remove the slide. During this process, the firearm discharged and the employee suffered a bullet wound to his left hand.

White River Energetics LLC

An employee was manually adding soda ash into a tank of heated water while on a mobile ladder. The fluid in the tank began to boil over the sides. The employee went to exit the mobile ladder to avoid the boiling liquid that was beginning to contact their lower legs. The employee lost balance and fell approximately 4 feet to the concrete below. The employee was hospitalized and had surgery for a dislocated left knee and thermal burns to the lower portion of both legs.

An employee was changing the bit on a horizontal gun drill. The wrench slipped while tightening a collet and the employee's left little finger was fractured and partially amputated.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
