An employee was operating an oxygen-deficient oven that required manual ignition to make charcoal from wood. As the oven was opened, oxygen was introduced to the environment, resulting in an explosion. The employee sustained head trauma as well as fractures to the arms and legs.

The employee went to the sand-removing unit/trailer to check if the well was flowing. He was standing on the steps and walkway of the trailer when an explosion occurred that knocked him over the walkway handrail resulting in a 3-4 foot fall. He was hospitalized with a fractured left foot and first-degree burns to his hands. A generator about 37 feet away had caused the ignition.

They unscrewed the top of the machine to retrieve the stuck samples when a small amount of pressure caused liquid to squirt out and onto one of the employee's face shields as the lid was removed. The employee was hospitalized due to chemical exposure.
