On August 13, 2025, an employee was marking out line and grade for an inlet box. While he was stepping from one 18-inch reinforced concrete pipe to another, his foot slipped and he struck the pipe and fell. He suffered a broken left tibia.

On March 28, 2025, an employee working as a machine operator was changing the teeth on a milling drum. He was using the tooth extractor when his left index finger became caught between the drum and the extractor. The employee's fingertip was amputated.

A traffic control crew was picking up road barrier barrels and loading them onto a trailer. A box truck rear-ended a police car, struck the traffic control trailer, and struck two traffic control employees. One of the employees was hospitalized with a severe laceration to their liver and fractures to their right tibia and fibula, as well as their left arm.

An employee was pulling the joint off a southbound lane and checking the depth. The employee was walking on the right side of the paver when a vehicle swerved into the closed lane and struck the employee, launched him into the air, where he struck a spreader. The employee sustained a contusion to the body, a right arm laceration, a fractured right tibia, and a right leg laceration. The driver of the vehicle was arrested for driving under the influence (DUI).

On October 26, 2023, two employees were performing construction work when they engaged in a physical altercation. One employee was hospitalized with a brain bleed and a back injury.

On December 19, 2023, an employee was setting up a copper wire drawing machine when his left ring finger was pinched between a copper wire and a machine part. The employee's finger was partially amputated.

An employee was examining a machine when a part moved up and amputated a fingertip on their left hand.

An employee was checking the gusset seal on a machine. He moved to the next cross sealer and the machine activated, causing an amputation to a left-hand finger. The employee was hospitalized.

An employee was evaluating a coiling machine to verify that the blade was present inside it. The machine's weld function was activated, and the machine crushed the employee's fingers, resulting in the amputation of the left middle and ring fingers.

An employee's right hand was caught in a wire drawing machine, resulting in amputation.

An employee was stopping traffic at a job site so a piece of equipment could be moved to another location when he was struck by a car. It sent him in the air and he landed on his back, resulting in hospitalization with five fractured ribs, a fractured forearm, and a liver injury.

A truck of asphalt binder was being unloaded. An employee was checking the transfer hose connection on the truck. The gasket on the hose blew out, causing the asphalt binder to shoot out and burn the employee's face, neck, and arms.

A truck driver was on a platform, pulling a tarp on an asphalt load in a six-wheel dump truck. He fell from the platform to the ground and suffered broken ribs.

An employee was using a cutting torch to heat up bolts that had seized up. The bolts caught on fire in a flash burn, and the employee suffered burns to the arms and elbows.

On May 15, 2025, an employee was cutting down steel mesh and debris that was protruding from a milled roadway. As the employee began to cut a piece of steel with an angle grinder, he steadied himself with his left hand and the tool contacted the top of his left wrist, resulting in a laceration that required hospitalization.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
