At about 12:00 a.m. on July 19, 2021, an employee had just cut through an aluminum part on a bandsaw when the blade lacerated the employee's right little fingertip.

An employee was lifting a die plate with a hoist. The employee's finger was caught between the hoist hook and the die plate eyebolt, and the fingertip was amputated.

On September 22, 2016, an employee was standing on the stairs next to a forklift that was prying solidified metal off a metal casting. The forklift freed the metal and it struck the employee in the chest and face, causing the employee to fall 4 or 5 feet backwards over a guardrail. The employee was hospitalized.

While performing a maintenance task on a mold tilting device, an employee's hand was caught in the device, causing muscle damage that required surgery.

On or about April 28, 2015, an employee sustained a serious injury to the left hand while changing the RLCH chop saw blade located in the finishing department. The saw was not locked out at the time of the incident.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

An employee was adjusting the oscillator pressure injector on a die-cast machine when his right thumb contacted the adjustment handle. The machine oscillated and closed on his right thumb. The tip of the thumb was pinched and amputated between the die and the handle.

An employee in the melt department was getting ready to put a recirculation pump into the pump housing area. He first put the pump in a cabinet to warm it up to avoid thermal shock. It shifted and the cold refractory contacted the hot metal. Molten metal contacted the employee, resulting in third-degree burns between their knee(s) and ankle(s). The employee was wearing some personal protective equipment (PPE) at the time but this was breached by the molten metal.

On May 15, 2025, an employee was assisting with troubleshooting an issue with a die in a die casting machine. While unjamming the machine, the employee's finger was caught between a section of the die and a wooden spacer. The employee's left middle fingertip was amputated.

An employee was checking the die casting machine when the cavity access door closed behind him and the employee sustained eight fractured ribs. The employee was hospitalized.

An employee was using a vertical band saw to cut a piece of square metal tubing when their left hand contacted the saw blade and their middle finger was amputated.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
