An employee was working from a 10-foot platform, tripping tubing, when the elevators made contact with a stub on the next joint of tubing. The employee dismounted the platform and landed on the ground 10 feet below, breaking both legs.

Employees were installing well head equipment. An employee's left middle finger was caught between a bolt and a flange that was being installed. The finger was amputated.
An employee was in a forklift that was stuck in mud. A truck was pulling the forklift with a chain to get it out of the mud when the chain broke and snapped, hitting the employee who was in the forklift. The employee suffered fractured ribs and a fractured shoulder blade.
