An employee was straightening bricks on a jig for the shipping monorail. As the strapping machine on the monorail rotated, the employee's head was caught between the jig and the machine, fracturing her jaw.

On July 7, 2016, at 0930, an employee was reaching into the strap station and jig of the brick strapping machine to straighten up the stack of bricks before they were banded. At the same time, the operator activated the machine, causing the carousel of jigs to move forward. She was pulled into the machinery, crushing her upper torso and head between the jig and the strap station.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

An employee was operating a transfer car when it stopped moving. He noticed that a brick had fallen and jammed. When he went to remove the brick with a shovel, the car started moving and caught his left foot against the pit's concrete wall. He suffered a fracture and lacerations to the lower left leg.

On July 2, 2025, an employee was walking to a building through a shed used to store dirt and went to climb a 45-inch pile of loose clay material when they tripped and fell forward. The employee sustained a dislocated right ankle, torn ligaments, a fractured tibia, and sprains to the right ankle and leg. The employee was hospitalized.

An employee was operating a stationary masonry saw, cutting faces from bricks for sample boards. The saw blade lacerated the employee's right forearm and the employee was hospitalized, requiring surgery.

An employee was removing a jammed ceramic tile from the stacker line and broke the jammed tile with another tile. The tile shattered and a sharp piece remained stuck on the belt, lacerating his arm. The employee sustained a severe laceration above the wrist and nerve damage that required surgery.

An employee was removing debris/material from the door of the mixer when their right hand was caught in the auger of the mixer. The employee sustained right index and middle finger amputations from the paddle of the mixer.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
