An employee was cutting sanitary pipe when he began to feel overheated and experience a headache and difficulty breathing. He then sat in the air conditioned work truck to cool down until he returned to work. The employee was carrying tools when symptoms reoccurred and he began to experience muscle cramping, resulting in hospitalization for heat illness.

Employees were installing an intermediate driveshaft in the driveline of a dump truck. One employee went under the truck to visually observe the yoke and their left thumb was crushed/lacerated in a pinch point between the universal joint cap and the yoke.

An employee was utilizing a battery-operated circular saw to cut a 6-inch back-form board when the saw blade struck their left hand, resulting in lacerations to the middle and ring fingers.

A crew was rigging an RCP pipe to be lifted by a backhoe. The injured employee signaled for the operator to lower the pipe. He was adjusting the choker cable on the pipe when the pipe was lifted again. His left index and middle fingers were then crushed between the cable and the pipe, resulting in fingertip amputations.

An employee was preparing to connect storm pipe to a structure. The employee went to connect the choker cable to the excavator bucket and the pipe slipped and struck them. The employee sustained a crush injury to the left foot that involved multiple fractures.

An employee was pulling and spraying weeds in the median of a road. A car ran off the road and struck the employee breaking his left leg.

An employee was placing sod alongside a roadway. After being hit by a car mirror, the employee jumped back and fell into the path of a skid steer that was backing up. The skid steer ran over his legs, breaking both of them.

On September 18, 2020, at 6:21 AM an employee working on the rear passenger side of the garbage truck was struck by a vehicle while picking up curbside garbage. The employee suffered a broken orbital bone, broken cervical discs and facial lacerations.

An employee was loading packages into the right side of a mail truck when a garbage truck struck the left rear-end of the mail truck. The impact pushed the mail truck onto the sidewalk and into a building, and the employee was thrown from the truck to the ground. The employee suffered collarbone and head fractures.

A vehicle struck an employee while she was walking across a parking lot, breaking her right shoulder and her tibia.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
