Final narrative
While running to stop a truck that was moving without a driver, an employee fell and broke his hip.
Two employees were working on a boom truck that had outriggers and a leaking hydraulic cylinder. The injured employee had removed the hydraulic cylinder, rebuilt it, and reinstalled it. While verifying that the repair was correct, a bolt fell into the frame rail. As the injured employee reached to retrieve the bolt, the hydraulic cylinder went into motion resulting in amputation of the first knuckle from the right index finger.
An employee was working on a truck at a fuel station when the truck back-fired and ignited the gasoline vapors, causing second degree burns to both of the employee's arms.
