On June 5, 2025, at approximately 6:45 AM, an employee was installing an auger tube onto a combine when it wouldn't slide into place due to a bind. After adjusting the hoist to remove the bind, the auger slid into place and struck their left thumb. The employee sustained a partial amputation of the thumb to the distal interphalangeal joint.

An employee was bending a part on a truck brake machine. The employee suffered an amputation to the right index finger at the first joint below the nail.

An employee was removing a pump assembly from a machine. The employee's finger was caught between the pump and the machine, and the employee suffered a fingertip amputation.

An employee was connecting a powered industrial truck (PIT) winch hook to steel plates. The winch engaged and the employee's left hand was caught between the hook and the steel plates resulting in amputation of their left middle fingertip.

An employee worked an entire shift and sustained heat stroke.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee was welding lift arms when their shirt caught fire due to welding sparks or heat, resulting in thermal burns to their upper chest.

On September 16, 2025, an employee went to pull a weld stand when he tripped and fell, landing on the floor on his right hip. The employee suffered hip and fibula fractures that required hospitalization and surgery.

An employee was accessing a tensioned hook that required a hoist mechanism to be engaged. He was unhooking the lift hook from a shelf column when the "up" button was pressed, which released the tension on the pneumatic air hoist. The employee's right index finger was caught between the hook and the column, resulting in a soft tissue amputation and fracture to the fingertip.

A crane was lifting a pole out of a zinc bath in the galvanizing department when the chain attached to the pole broke. The pole fell back into the bath and liquid zinc splashed onto an employee, who suffered third-degree burns to an arm, the abdomen, the torso, the upper left side of the chest, an ear, the neck, and a hand.

An employee tripped over a bin and fell to the floor. She landed on her hip and braced herself with her arm, resulting in a fractured hip and wrist.

An employee was diagnosing the lack of flow of product to a powder bin. The employee removed the rotary star valve below the bin. While he was reinstalling the valve, his right middle finger was crushed between its shaft and its housing. The finger was partially amputated.

The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.
