Final narrative
On or about August 2, 2016, an employee was sawing pieces of plastic with a band saw when the blade lacerated his left-hand index finger, requiring hospitalization.
An employee was leveling out a mold when the employee slipped on hydraulic fluid and fell on a press bolt, injuring the upper right arm and requiring hospitalization.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
