On Thursday, August 4, 2016, at 11:30 a.m., an employee was working outside, marking a new engineered path with flags for an underground service drop from the electrical pole to a relocated electric meter at a new residential construction site. As the employee was walking toward the electrical pole, the adjacent ground grading changed; it was partially graded and partially undisturbed natural landscape, creating an uneven walking surface. Subsequently, the employee's right ankle rolled and she fell to the ground, fracturing her ankle. She was hospitalized overnight for surgery.

Two of the employees climbed on top of the crew's truck to fix the issue. The load then pulled the winch line tight around the finger of one of the employees, causing the amputation of their fingertip.
