An employee had just cleared a jam in a conveyor when he turned left to walk away from the conveyor while his right hand was still resting on the guard. His right hand slipped and made contact with the conveyor, and his right pinky and ring fingers became caught between the roller and the fixed roll. His right ring fingertip was amputated, and his right pinky finger was fractured.

At 7:15 AM, an employee was operating a refiner and was on the operation control end with his hand on the quick-release lever. The lever was in the up position when the quick release became disengaged and the lever came down on the employee's hand with force. The lever amputated his left thumb just above the second knuckle.
