Final narrative
At 7:15 AM, an employee was operating a refiner and was on the operation control end with his hand on the quick-release lever. The lever was in the up position when the quick release became disengaged and the lever came down on the employee's hand with force. The lever amputated his left thumb just above the second knuckle. He was hospitalized.
A technician was in training, performing a check on the wheel abrator. He opened the door on the enclosed equipment to visually inspect and noticed debris at the bottom of the machine. He went to clean out the debris and his left middle and ring fingers made contact with the rotary blades resulting in amputation of the fingers and hospitalization.
