Pratt Industries

The injured employee was manually moving a belt into position on a four belt gear pulley. The machine was locked out at the time. While walking belt #1 over onto the pulley where belt #2 had broken off, the injured employee had a co-worker manually rotate the shaft on the other side of the machine to help walk the belt into position. The injured employee was pulling up on the belt with his right hand and was using a screwdriver with his left hand to move the belt. The rotating shaft then reversed direction and the injured employee's right-hand fingers were caught between the belt and pulley resulting in a partial amputation to the middle fingertip and a fracture to the pinky finger.

The employee sustained fractures to their hips, arm, and shoulders. They also sustained torn rotator cuffs in both shoulders. The employee was hospitalized and required surgery.

An employee was climbing a hook ladder. A piece of advertising material fell onto the employee, causing them to slip and fall from the fourth or fifth rung. The employee landed on their left arm, breaking the arm and also suffering a broken rib.

An employee set up and secured an extension ladder on a pole so he could tighten up a low-hanging triplex cable. While on the pole, the employee started to pull the triplex cable to raise it up.
