An employee's hand was caught in a machine's pinch point.
On January 20, 2020, an employee was at a residence removing a fan unit from the sidewall while on a scissor lift. He lost his balance and fell 8 feet from the lift to the floor, hitting his head. He suffered several facial and head lacerations and a skull fracture.
A rigging crew was adjusting the straps of an 800 lb. steel beam member being lifted as part of a steel building construction when the beam clamp slipped and the beam fell, striking an employee who was checking rigging nearby on his back. He suffered broken ribs and lacerations.
An employee was standing on the ground using an overhead crane to move small light beams from a conveyor to a
staging area when the stack of beams became unstable and started to fall over. As he attempted to get out of the way of the falling beams, he fell to the ground, dislocating/fracturing his left elbow. He was hospitalized and had surgery.
An employee was putting a strap around a steel beam when another beam shifted and fell on his right hand, fracturing his right wrist and breaking fingers.
