Employees were working inside a chemical processing reactor, lowering a tray panel to a lower level. The tray panel fell 18 to 24 inches and struck the left foot of an employee who was removing the rigging. The employee sustained a big toe fracture.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
