Final narrative
An employee was cleaning the filling machine and emptying it of liquid. She then attempted to see if there was any remaining liquid. Her left index finger was inside a quarter-sized hole where a piston was when the machine activated, causing the piston to move and amputate approximately 1/8" of her left index fingertip. The machine's air source was connected at the time of the incident.
Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
