On December 12, 2018, a maintenance technician was performing piping work on a rail car when approximately 85 pounds of chlorine gas was released. Another employee in the area breathed the chlorine and was hospitalized for acute respiratory problems.

Her left index finger was inside a quarter-sized hole where a piston was when the machine activated, causing the piston to move and amputate approximately 1/8" of her left index fingertip. The machine's air source was connected at the time of the incident.
