An employee was removing a pipe, which was about 1 foot above the ground, when the pipe fell on the employee's right foot and broke toes.
While a crew was completing a casing connection, which involves pieces of pipe being torqued together, an employee's fingers were caught in the top drive wheel. The employee's right middle finger was amputated, and the index and ring fingers were lacerated.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
