The employee's cheek made contact with a connector with damaged insulation. The employee was shocked, briefly lost consciousness, and fell to the ground, suffering an injury to the left shoulder.

Stallion Oilfield Construction, LLC

An employee was removing a hot water heater (that was full of water) from a trailer. The glove on his left hand was caught on the pipes of the moving water heater, causing him to lose balance and fall approximately 4 feet from a ladder to the ground below. The employee's lower left leg and hip were fractured.

Employees were upgrading lighting fixtures in the accounting offices. The injured employee was on a ladder working on a ceiling light fixture. He came into contact with the metal grating surrounding the fixture and sustained an electrical shock that caused him to lose balance and fall approximately 3 to 4 feet to the floor. The injured employee sustained a fractured pelvis and a torn rotator cuff.

Electrical Specialists, Inc.

An employee was working in an electrical junction box and was shocked by an energized circuit. The employee fell off a ladder about 3-4 feet to a cement floor, sustaining fractures to his left shoulder and right heel. The employee was hospitalized.

The injured employee loaded a container onto a railcar and then mounted the railcar to put in the locking mechanism for the container. The employee was struck by another container that a crane was loading onto the railcar and fell approximately four feet from the railcar to the concrete ground. The employee sustained fractures to their pelvis, shoulder, and two ribs.

An employee was performing rebar fabrication using a bending machine. The employee's glove became caught in the area where the rebar bend overlaps onto itself. The subsequent bending/cutting caused the two overlapping portions to squeeze together and amputate approximately 1/4" of his left index fingertip.

An employee was building wood detail blocking and a wood member needed to be resized. The employee took the wood member to the table saw and set the fence to the correct size. The employee pressed down on the board to control it when his left little finger contacted the running blade, resulting in an amputation.

An employee was clearing a pipe along a culvert using a motorized plumbing machine. The end of the machine got tangled and stuck in long grass and other debris. The employee went to lift the end of the machine out of the grass and clear the blockage when his hand was pulled into the machine, resulting in a partial amputation and open fracture of his left distal phalanx.

On July 29, 2025, an employee was transporting 10 sheets of drywall on a cart. While walking down a ramp the employee felt discomfort in his calf and then lost consciousness due to heat. The employee sustained severe dehydration and an injury to his calf.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.
