An employee was using a 6 foot ladder to install a membrane on the side of the building when he became unbalanced and fell backwards, striking his head on the ground.

On December 11, 2015 at approximately 11:00 AM, a staffing worker was asked by the host company to get on the roof to help take off fiberglass panels. The employee stepped on a weak spot and fell through twelve feet breaking his right femur and sustaining other injuries to his spine and ribs.

An employee was riding on the back of a trash truck vehicle when the truck struck a vehicle while reversing. The truck jerked as it shifted gears to move forward, causing the employee to fall from the back of the truck to the pavement. The employee sustained a broken shoulder.

On September 2, 2015, an employee was mopping/cleaning a church floor when she slipped and fell on the wet surface, striking the back of her head on the floor. She sustained serious head injuries and required hospitalization.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
