Final narrative
An employee removed a guard from a conveyor belt motor to check the chain and sprockets for tension. The employee reached toward the running chain and sprocket and his left thumb was caught between the chain and sprocket resulting in an amputation of 1/4 inch to 1/2 inch of the thumb. The machine was energized at the time of the incident.
An employee was operating a forklift to travel between buildings. As the lift was moving down the ramp, they reached out to press the open button for the overhead door. While pulling their arm back into the lift, the lift started to move down the ramp, catching their arm between a bollard and the forklift frame resulting in a left radius and ulna fracture.
