An employee was loading a dye header (a piece of carpet) into a sample beck. When she tried to pull the head end of the dye header from underneath the reel, the carpet pulled her arm between the reel and the carpet, breaking two bones in her left forearm.
An employee was repairing the tufting machine and straightening up the yarn on a puller roller when his right ring finger was caught and amputated in the machine.
